40-50%的患者血清心肌酶谱无典型改变 尽管WHO（世界卫生组织）、各国心脏病学会、心血管疾病的教科书都明确指出，胸痛是AMI（急性心肌梗死）的最典型症状，但仍然不能忽略：还有33%的 患者不表现胸痛症状这一事实。同样的，40%的患者并不表现大量出汗症状；40%的患者ECG（心电图）检查不表现诊断性特点；40-50%的患者血清心 肌酶谱无典型改变。 有谱没谱 那么一度作为心肌损伤诊断“金标准”的心肌酶谱，到底有谱还是没谱呢？让我们细细来看。 心肌酶谱，是指存在于心肌细胞中的所有酶类的总称，当心肌细胞发生破损时，这些酶类进入血液循环而被检测到异常变化。临床上以检测心肌酶谱五项（主要为细 胞能量代谢或合成代谢必须的酶类）为主，主要包括乳酸脱氢酶（LDH）、肌酸激酶（CK）、肌酸激酶同工酶（CK-MB）、谷草转氨酶（AST）、α-羟 丁酸脱氢酶（α-HBD）等五项。 这些酶中，LDH与AST广泛存在于身体各个组织器官中（心肌细胞中的含量并不高），因此特异性较差，检测结果易受其他组织器官损伤的影响。另外三个酶类 虽然心肌中的含量相对要高一些，但同样在其他组织器官中并不少量的存在着。心肌酶谱五项中，以肌酸激酶同工酶（CK-MB）对心脏的特异性是最高的。在没 有其他检测手段或标志物的年代里，心肌酶谱尤其是其中的CK-MB一直被视为诊断心肌损伤类疾病的“金标准”。 肌酸激酶（CK）是由两个亚单位结合而成的二聚体，M亚基为肌肉型，B亚基为脑型，共形成了三种同工酶。CK-BB主要存在于脑组织、胃肠道及子宫平滑肌中，脑组织中几乎全为CK-BB，CK-MB主要存在于心肌组织中，CK-MM主要存在于骨骼肌组织中。 生化检测中的CK-MB常采用免疫抑制法：即用抗CK-M单体的抗体将M亚基完全抑制，所以CK-MM会失去活性，而CK-MB活性会失去一半，这样测出的活性实际就是CK-MB的一半，所以CK-MB的活性应该为测定值的2倍。 很显然，对于脑部疾病患者、胃肠道疾病患者来说，其血清中的CK-BB会使得测定结果假性升高；对于肌肉损伤
